Blood pressure chart
The new blood pressure meter I purchased did not
come with a supply of ready to fill in forms. The old one had some
forms but there was no provision for recording the pulse rate that
the new meter provides. I used this 24 hour 7 day chart for the first
month or so then changed to a daily 31 day chart.
